Summary: St. Anthony North's Radiation Oncology team is looking for a locum to help fill dates need for their providers vacation dates. Shifts are 8a-4:30p. Site will have sporadic ongoing vacation coverage dates needed as they become available.
BID DUE DATE: Dec 6th at 5p EST!


Rate Caps for Submission:
Malpractice - Must be included in rates.
Orientation - half the hourly/callback rate*** kicks in after 8 hours. "Per hour of orientation or training beyond 8 hours"
Hourly - *** and below - "Per hour, not OT"
Holiday -Must be 1.5x the regular rates "Per holiday hour, no OT"

Schedule/Availability Requirements:
All shifts are 8a-4:30p
Feb 6-7
Mar 24-28
April 17-18
ongoing dates will be needed for sporadic vacation coverage.

Facility Location: 14300 Orchard Pkwy, Westminster, CO 80023
Setting: Outpatient
FTE: 0.01
Days per month: Sporadic dates of coverage as the team needs
Required Procedures: Radiation Oncology, Emergent/Urgent clinic Visits and Treatment Planning, Treatment Supervision.
EMR: EPIC, Aria
Machine is a Varian True Beam. Treatment planning is in Eclipse.
Support Staff: RN
Reason for Coverage: Gaps in coverage.

EMR Required Training:
Module (learn lessons) - Must be completed prior to virtual EMR training to attend the training.
EMR Virtual training - Only able to attend if Modules are completed before training. Virtual training must be completed prior to the providers first day.


Travel Preferences:
Preferred Lodging: Preferred hotels
Air Travel: Air travel okay.
Please detail any anticipated travel exceptions, for example a dietary restriction necessitating specific lodging situations. Travel exception requests will be denied if this information is not part of the provider presentation. Please also indicate the city where provider will be traveling from in the travel section of the provider submission.

Credentialing Information:
Timeframe: 60-90 days is standard.
Emergency temp privs? Yes with clean file only, 1 week minimum needed.
Credentialing with other facilities? No
Billing enrollment - Is this position providing services in lieu of an existing enrolled provider for a period of 60 days or less? Yes

Admissions - New clinic, volume is being established.
SUBMISSION REQUIREMENTS
Board Certification Radiation Oncology - Required
Active CO license - Required or IMLC (LOQ in hand at time of submission)
Candidate must be available for most of the dates detailed in the job description to be considered - Required
Must acknowledge candidate understands the EMR training requirements set forth in job description and will complete in this order to avoid missing their start date - Required
Clean MP/background - Highly Preferred, site will place priority for candidates with a self inquiry NPDB at time of presentation.
